  • 1. Submit Your Request
    Complete the consultation request form with basic information about your loved one’s situation and how to reach you.

    2. Case Review
    Your request will be reviewed to determine whether an advisory consultation would be appropriate and helpful.

    3. Consultation Confirmation
    If the consultation is accepted, you will receive an email with availability and a secure payment link to reserve your consultation.

    4. Record Review and Preparation
    You will have the opportunity to share any available medical records or summaries before the consultation so they can be reviewed in advance.

    5. Consultation and Follow-Up
    We will meet by phone or video to discuss your loved one’s situation, clarify medical information, and answer questions. Follow-up questions are welcome later the same day.

    • Review initiated within 2–4 hours of intake

    • Briefing call by phone or secure video, scheduled promptly thereafter lasting 30-60 minutes depending on the need.

    • Ongoing communication during the 12 hour consultation period, as appropriate.

    • ICU / Critical Illness Advisory Day — $350

    • Additional days during the same admission — $250

    • Chronic Care / Record Review — $350
